Apply with NY State of Health (NYSOH)
Apply for coverage by visiting the NYSOH website or by calling the NYSOH Consumer Service Center at 1-855-355-5777 (TTY 711).
Note: Your application must be processed by the 15th of the month for your coverage to begin on the first day of the next month. For example, if your application is processed on January 15, your coverage will begin on February 1. If your application is processed on January 16, your coverage will not begin until March 1.
