What is Active Anytime®?
Active Anytime® is a flexible fitness benefit available exclusively to most Medicare members with Univera Healthcare coverage. Active Anytime® provides you with up to $650* per calendar year to use toward qualified fitness facility and weight management program membership fees.
May I participate when I travel?
Yes, but you may be required to submit a paper claim form.
How does it work?
First, start by selecting a qualified fitness facility or weight management program you would like to join. Our Active Anytime® card is not a credit card, but is compatible with MasterCard processing systems for purposes of utilizing your Active Anytime® benefit to pay your membership fees. Ask the facility if they accept MasterCard as a form of payment. If the qualified fitness facility or weight management program does accept this form of payment, you should request (order) your Active Anytime® card for use at the qualified facility.

What if the program I’ve selected cannot accept the Active Anytime® card?
You will need to pay for your membership fee up front and then submit a Active Anytime® claim form (PDF).
* This benefit does not cover any ancillary services or items that are not part of a membership fee. Any unused portion of your $650 fitness benefit will not be carried over to future coverage years. Benefits may change on January 1, 2012. Please contact us for details.
Univera Healthcare contracts with the federal government and is an HMO plan and PPO plan with a Medicare contract. Enrollment in Univera Healthcare depends on contract renewal. Submit a complaint about your Medicare plan at www.Medicare.gov or learn about filing a complaint by contacting the Medicare Ombudsman. Y0028_3568_0 Approved
