New Group Application

The Department of Financial Services (DFS) has required us to consolidate the documentation required to implement a new group into one application.  Effective with submissions received July 1 and beyond, this application must accompany each new group submission. 

  • For small groups, all applicable checklist items need to be included with the application. 
  • For large groups, all applicable checklist items need to be provided to obtain the experience rated quote. If the group accepts the quote, the application can be submitted with the signed rate sheet.
