If you’ve become the Broker of Record for a new account, follow the steps below to send us documentation of this permission. Once received, our Commission Administration Team will process the request, which will take effect the first day of the following month.

Step 1: Get the Employer Group's Authorization
  Ask your client to draft a letter giving you permission to act on their behalf. The letter must be prepared using the employer group's letterhead. Use this Broker of Record Letter Template (PDF) for suggested wording.
Step 2: Submit the Letter to Us Electronically
  Follow this link to submit Broker of Record Documentation.

If you'll also be performing Online Enrollment and/or Daily Updates, have the group complete this additional form for you:
